The Membership Program provides patients with consistent access to aesthetic treatments, preferred pricing, and exclusive benefits. By enrolling, members agree to the following terms and conditions.

Membership Fee & Billing
- Monthly membership fee: $139
- Billing is processed each month automatically on the enrollment date.
- A valid credit card must always remain on file.
- Membership benefits begin once the first payment is successfully processed.
- Valid at all TPSG locations
Membership Commitment
- A minimum commitment of 6 months is required upon enrollment.
- If there are payment interruptions during the 6 months, or if you cancel before the commitment period, you will be financially responsible for the balance of the minimum period
- After the initial term, membership will automatically continue on a month-to-month basis unless canceled in accordance with this policy.
Accounts
- Are non-transferable
- May only be used by the enrolled member

Radiance Rewards Passport

Each member receives one stamp for every Med Spa visit or qualifying purchase. Touch-ups are excluded. Rewards earned cannot be combined or used simultaneously. When members reach milestones, they unlock specific rewards. (The program will be reset once 15 visits have been reached)

IMPORTANT INFORMATION

Membership Policies & Terms

Cancellations & Early Termination

Membership may not be canceled during the initial 6-month commitment.
After the initial term, members may cancel with 30 days’ written notice (via e-mail or patient portal) or in person with a valid driver’s license to the practice.
If a member chooses to cancel before fulfilling the 6-month commitment, the member will be responsible for paying the remaining balance of the membership.

Re-Enrollment Policy

Members who cancel their membership are not eligible to re-enroll for a period of 3 months from the cancellation date.
Documentation may be required.

Missed Payments / Failed Billing

If a payment fails:
- The member will be notified and given 7 days to update the payment
- Benefits are temporarily suspended until payment is resolved
Accounts more than 30 days past due are subject to cancellation and re-enrollment restrictions and will be billed for the remainder of the membership if still within the 6-month minimum period.

Refund Policy

Membership fees are non-refundable
Membership payments: If you cancel in the middle of the month, the official cancellation date will be at the next billing period.
These funds:
- Cannot be redeemed for cash
- Cannot be transferred to another individual
- Must be used for eligible services only

Appointment & Cancellation Policy

Standard TPSG cancellation policy applies:
- 48 business hours’ notice required
- Late cancellations and no-shows are subject to applicable fees
Membership does not waive cancellation or no-show fees

Use of Benefits

Membership benefits:
- Are non-transferable
- May not be shared between patients
Treatments are subject to provider evaluation and medical appropriateness.

Program Changes

The practice reserves the right to:
- Modify membership pricing, benefits, or terms with 30 days’ notice (via email)
- Update eligible services or exclusions as needed

Medical Membership Freeze

During the initial six-month membership commitment, a member may request one medical freeze of up to three consecutive months due to a qualifying medical circumstance, including but not limited to pregnancy or a significant health diagnosis or medical condition that temporarily prevents the member from utilizing membership services.

A medical freeze requires documentation from the member’s treating physician or qualified healthcare provider and is subject to TPSG approval.

During an approved medical freeze:

Monthly membership billing will be temporarily suspended.
Membership benefits, discounts, rewards, and promotional privileges will also be suspended.
The freeze does not count toward completion of the six-month minimum membership commitment.
The remaining membership commitment will resume when the freeze ends.
The maximum freezing period is three consecutive months.
Only one medical freeze is permitted during the initial six-month commitment unless an exception is approved by TPSG administration.

Termination by Practice

TPSG reserves the right to terminate a membership for:

Policy violations
Abusive behavior toward staff
Misuse of membership benefits

Agreement

By enrolling in the Membership Program, the member acknowledges and agrees to all terms and conditions outlined above.
